Patient Safety

Our community has placed their trust in us as healthcare providers to care for them and keep them safe. Unfortunately, nearly 1 in 5 people have either been harmed personally, or know someone who has been harmed while under our care. Healthcare providers are intelligent, passionate people who come to work every day to provide excellent care to their patients. Creating a world class patient safety program requires a focus on the system of care delivery that is reliable and prevents errors and is built upon a transparent culture of safety where we learn from our errors. Our team will partner with you to assess your current patient safety program, analyze your safety culture survey results, and provide teamwork and communication training that is based on the programs developed by airline professionals.

Care Management

Healthcare organizations are becoming more accountable for the patient’s health along with their illness. We have learned that even though we have provided great care and “checked all of the boxes” on the discharge checklist, patients leave our health care institutions confused and many times without the resources or support needed to maintain their health. Care Managers not only need to understand the health plans benefit structure but also consider what happens to the patient when they are discharged and transition back to their home or community. Care Transition programs have opened our eyes to the reality outside of hospitals and physician offices where patients lack transportation, fail to fill prescriptions, do not receive services that were ordered by their physician, such as durable medical equipment or physical therapy. MCB has worked with several healthcare organizations to redesign their case management services to ensure safe transitions to the next level of care and mitigate the gaps that contribute to readmissions or other poor outcomes and increase the cost of healthcare.

Patient & Family Engagement

Engaging patients and their families in their care requires a committed partnership between providers of care and the patients and families with whom they interact. Understanding what is meaningful to patients and listening to their perspective is enlightening and rewarding. Increasing patient engagement can yield many benefits, including improved health and outcomes for individuals and more productive work environments for health care professionals. Many tools currently exist that can be used to conduct assessments, identify opportunities and develop a program, but training is needed both on the part of clinicians and patients.
